Surgery Instructions & Consents

Welcome to the Pre and Post Op & Page.  This page is designed so that our patients can have 24/7 access to our surgical forms.  This means that it is possible to read and print out your necessary forms from the convenience of any computer anytime!  The forms are extremely important and can have a lot to do with the success of your surgery. You will be asked to print out the consent and instruction forms specific to your surgery or surgeries, read them and mark any areas of question or concern.  You will need to bring the forms to your pre op appointment and sign them there with our staff.  Please DO NOT sign the forms before coming to the office.  You can initial the specific boxes and complete everything except the signature as it must be witnessed.

• The PREOP FORM (A1) form explains what is necessary for most basic surgeries, especially those requiring IV anesthesia.

• The CONSENT FORMS present the common presurgical consent process for most of our common surgical procedures.  You will be given the number of each form that you need to print out and read.  Please read these forms very closely and mark any questions or areas of concern prior to your preoperative appointment so we can go over these specifics in detail.  It is not uncommon that these forms are unsettling and will scare some patients.  They are not meant to be pessimistic or scary and most doctors around the country use very similar forms.  Most of the complications discussed are extremely rare, but possible.  NO SURGEON can guarantee any patient that they will not have a post surgical complication, but again, most serious complications remain extremely rare.  This is similar to saying that it is possible that you may be involved in a fender bender while driving to the office.  Most patients will not be involved in a fender bender, but it is possible.  Please keep in mind that Dr. Niamtu only does cosmetic facial surgery.  It is his profession and passion and not something he dabbles in.  He is prepared to treat and or refer surgical complications should they occur.  His surgical center is fully accredited by the AAAHC and his anesthesiologists are board certified with decades of experience.  He has fully equipped emergency equipment on premises and and exceeds many safety regulations.  His 27 years of providing facial surgical care and high standards are focused on patient safety and convenience in all phases of the surgical experience.
• The POST OP INSTRUCTIONS include the important steps to get the patient through the fastest, safest and most pain free recovery.  Again, you should read all of the forms pertaining to your surgery BEFORE your surgery and write down any questions or concerns.
