This page is designed so that our patients can have 24/7 access to our surgical forms. The forms are extremely important and can have a lot to do with the success of your surgery.
The CONSENT FORMS below are for our common surgical procedures. You only need to print out the consent forms that apply to the procedures you will be having.
It is not uncommon that these forms are unsettling and will scare some patients. They are not meant to be pessimistic or alarming and most surgeons around the country use very similar forms.
Most of the complications discussed are extremely rare, but possible. NO SURGEON can guarantee any patient that they will not have a post surgical complication, but again, most serious complications remain extremely rare. This is similar to saying that it is possible that you may be involved in a fender auto accident while driving to the office. Most patients will not be involved in an accident, but it is possible.
Please keep in mind that Dr. Niamtu only does cosmetic facial surgery; and he does it all day everyday. It is his profession and passion and not just something he dabbles in. He is prepared to treat surgical complications should they occur. Our surgical center is fully accredited by the AAAHC and our anesthesia providers are board certified with decades of experience. We have fully equipped emergency equipment on premises that is similar to all local hospitals and and exceeds many safety regulations. Dr. Niamtu’s 30 years of providing facial surgical care and high standards are focused on patient safety and convenience in all phases of the surgical experience.
The POST OP INSTRUCTIONS shown below include the important steps to get the patient through the fastest, safest and most pain free recovery. Again, you should read all of the forms pertaining to your surgery BEFORE your surgery and write down any questions or concerns. Please don’t sign the forms until you come to our office.

Please note that Dr. Niamtu does not routinely use drains on his facelift patients. If a patient does require a post surgical drain (which is the exception, not the rule) the video below shows how to care for the drain. This video can be disregarded for the average patient and can be viewed in the event a drain is used.
